anyone from pune please help for shipping a phone

hey guys a frend is trying to ship a phone to me
courier guys are asking for sales tax form 402A to orissa

i have recieved so many times from many places but never ever faced such an issue

it is usually required in west bengal but havent ever been asked for my place

if someone has some contacts with courier office in pune, then please pm me
need urgent help.....
