Lost PAN card, which form to download?

I lost my PAN card few months back, so need to apply for a new one, but I was confused after going to IT's website. Could you tell me which form to download? As I've heard there are different forms for applying new cards and lost ones.

Thanks boss. Didn't know things have gone this much 'techy' nowadays here!
Btw, just confirming the bold part, I need to click on Individual and submit at the very bottom of the page and in that form I won't manually select any box at left margin, right? The address is already ticked though, shall I keep it that way?
 
@ithehappy Yeah.. Address field is mandatory for every application, and will be updated to whatever address you are providing right now.

Thanks. Just tried filling the form up and all, but at last stage, when I enter that Citibank gateway page and put my Debit card or Credit card details it just redirects me to this page!

Credit/Debit card doesn't work?
After putting the Debit card details it should go to a password page like every other transaction I do but it doesn't, I think that's why the problem is happening!
 
^^ Probably some problem with your bank or the payment gateway. I faced a similar issue a few days back on ebay. It was with my credit card though. The transaction went through when I tried it the next day.
 
Don't think there is any problem with the cards/bank, as I tried Debit and Credit cards of different banks, maybe the problem is with their site. Will try tomorrow or at tonight. And yes, I had the same problem just about a month ago on eBay too, then it was solved.
Btw, when filling up the form there was an option to submit identity and address proof, and I selected some options there but how would I send those things? There is no option to attach any images!
 
@ithehappy Please read and re-read the instructions in their site fully. This is an important document and I'm sure you don't want to screw up anything.
IV MODE OF SUBMISSION OF DOCUMENTS

(w) The acknowledgement duly signed, affixed with photograph (in case of 'Individuals') alongwith demand draft/cheque, if any, proof of existing PAN, proof of identity & address as specified in the application along with any other relevant proof as specified (in Item No.IV - Documents to be submitted along with the application ) is to be sent to NSDL at 'NSDL e-Governance Infrastructure Limited, 5th floor, Mantri Sterling, Plot No. 341, Survey No. 997/8, Model Colony, Near Deep Bungalow Chowk, Pune – 411016'.
(x) Super scribe the envelope with 'APPLICATION FOR PAN CHANGE REQUEST-Acknowledgment Number' (e.g. 'APPLICATION FOR PAN CHANGE REQUEST-881010200000097').
(y) Your acknowledgement, demand draft/cheque, if any, and proofs, should reach NSDL within 15 days from the date of online application.
(z) Applications received with demand draft or cheque as mode of payment shall be processed only on receipt of relevant proofs and realization of payment.
(aa) Applications received with credit card / debit card / net banking as mode of payment shall be processed on receipt of relevant documents (acknowledgement and proofs).

Not trying to be rude, but can someone please tell me what document can be used to correct the fathers name on the pan card ?

its my mother's, and she doesn't have any documents that mentions her father ( lost in floods )
 
Thanks. So I see that even after submitting that online form, and after they debit Rs.96 from my account, I need to send the identity proof documents in that Pune address of theirs right? But what is the acknowledgement form duly signed affixed with photograph? Will I get that after everything goes successfully with that online form? If not where do I get that? It's a little unclear to me.

Yes.. You will get the acknowledgement form after making a successful payment.[DOUBLEPOST=1375031086][/DOUBLEPOST]@rads Am not really sure what can be done. May be a notarized or attested document stating the details might help.

Thanks once again.
Just what I thought, it's officially Monday and it finally succeeded. Ah!
But as I don't have a printer I will have to print it out tomorrow, so I saved it by clicking save acknowledgement but when I open the page the tick image boxes are gone and replaced with missing image icons! Will there be any problem. I have attached screen shots to clarify what I mean.
