Email from ICICI...is this a genuine one?

Today morning I received an email from ICICI bank and it was in my spam folder so I moved it to my Inbox.

I downloaded the form and it opened into new tab ans warned me about accessing a secured SSL page/ site.
I never proceeded towards login in with my credentials as always thought this can be some spam or fishing activity hence reporting here.

I'm sure many of you icici users might have got similar email so do check your inbox or spam/junk folders and report here accordingly.
 
This is what ICICI says;
Safe Banking tip:
Never respond to an e-mail which requires you to confirm, renew or validate your account or card details, even if it appears to have been sent from your Bank.
Do not access your Bank's website through links in e-mails. Check the URL in your browser before entering your user name and password.
 
Definitely a phishing mail.

Things that should confirm your doubts:

1) Spelling Mistakes (Your 'gird' values)
2) Banking sites always begin with 'https://' and are backed by a security certificate. Details of which can be see when you click on the certificate icon in the address bar
3) Banks never send any such mails to their customers asking them to log in

OP thats a fake email.

However I am a Nigerian prince in dire need of assistance to move 25 (twenty five) million dollars out of a state held bank account. Fake. Most browsers show the hyperlink when you hover over the icici link, so you can check that out. Mostly it will be something odd like icicibankonline.com (a url I got once and now is defunct) or icicibankonlinebanking.com etc.
