JDownloader is highly recommended. 
Also try Captcha Brotherhood (plugin for jdownloader) - Captcha Brotherhood. It is a sort of community of members where you earn credits by solving captchas for others. And then you can leave your computer to download stuff at night and others will solve your captchas (spending your credits) each time a new download starts.
This is really useful for file hosts like fileserve and hotfile which use Google's recaptcha technology. These can't be automatically detected and a real person is nneeded to solve them.
If you're okay with downloading at <150 KB/s, you don't have to buy premium account at all.
