CPU/Mobo Irql_not_less_or_equal Error

Whenever I tend to record audio from Windows Sound Recorder or ANY software for that matter, I am welcomed by the BSOD with "IRQL_NOT_LESS_OR_EQUAL" message. I cant even configure my yahoo messenger for voice chatting. The moment someone invites me for a voice chat, the BSOD pops up and "beginning dump of physical memory" starts. When I restart my pc immediately using the Reset button I get a message, something like "Disk boot failure, insert system disk and press Enter", so I have to press and hold the power button until my pc turns off and when i start it again, it works fine until i try to record something or configure my recording medium.

Its a P4 2.7Ghz with an Intel 915 and 512MB of RAM. A Dax modem is the only thing i've added on to the mobo.

Guys please help me out, its so frustrating everytime the BSOD pops up.

EDIT: I run XP with SP2.
 
is this with onboard audio ? try updating your motherboard drivers then, this happens mostly with incompatible hardware

also try disconnecting your modem and record audio and see if the problem still occurs.

@bottle - yes its onboard audio. Thanx for the link m8, t'was very useful.
@vignesh - Yes i am able to defrag and run chkdsk.

I think the best thing to do is to format my comp and install the OS and all drivers again.
 
Before you do format your system, check if the Device Manager is showing a exclamation/question mark symbol alongside the onboard audio device ( meaning something wrong with that device )
 
^^ I checked, everything is fine. No yellow/red exclamation/question mark. So i've got no other solution than to format my system right?
 
As bottle said it is a hardware conflict, mostly between your Modem and Onboard sound.

First thing, try inserting your modem in another/new/different PCI slot, also update your modem and onboard sound card drivers.

I think the modem and sound card are sharing IRQ's - to check it - just open device manager/view/resources by type - check the IRQ no's of modem and sound card.
