Graphic Cards LeadteK GTX 460 OC | Strange Graphics issue

Hello friends,
Like the topic says, I am facing a strange issue with my graphics card.
Scenario 1
-----------
When running my machine in normal mode with the Nvidia drivers installed, My screen shows white lines all over the screen (screen shot attached) and some times also results to BSOD.
But when using it in Safe Mode, It runs just fine with no issues for hours.

Scenario 2
---------------
After I uninstalled the Nvidia drivers, the PC runs fine in normal mode too with no issues for hours.

I also tried using an older driver i.e. 275.33 but to no help.
I am using Seasonic s12II 520W PSU.
Earlier I was using CM Extreme power plus 600. Did this PSU already damage my card?

Is this sign of my Graphics card been dead?? Or is there any solution to it.
Please help...

Gpu dead, i was having same issue with this gpu, got it replaced and everything began to work normally.

Can you please tell me it RMA process?? Did you also have the Leadtek card????

Got it replaced by aditya infotech last year.
