Windows Need a software which can store reports and print

I have a format (multiple) for different reports . Is there a softwre which can be customized so that blank spaces can be left for entries and i can store each of the reports under different names while keeping the master blank spaced report in tact ?

I thought i could create a pdf with blank spaces , then i thought i cant store it , or customizing it (like adding logo , spacing etc) was difficult.

I thought of word , then again how to make one master file , which wont get changed after we enter datas into it ?

Any software or online platform which can accomplish it ?

You can create fillable forms with word. You will be creating a template basically. This template will remain as it is, and after data is entered and you save it, you will be prompted to save it as a different document. Template will remain unaffected.

will it be possible to extract any data from the saved reports later ? i mean say age , sex , location kind of info ? Also will there be option to say enter certain data , but not print it ..

I mean say i want ph.no , which i want it on report for my use , for later reference , but dont want it to be printed ..

Quickbooks might do needful for sure..
It can print automatically and do various needful things, you have to check out feature list and your requirements.

There is yet another tool, but not as professional like mentioned, as i recollect i will answer.
