Haven't played Quake 4 in a very long time but I don't recall having any problems with it. Sounds like the save game file that you are using could be corrupt. Try starting the game from a differant save game file.
If that doesn't work use a cheat to direclty load the second level.
Is it a original or a pirated version, because if its pirated it could also be a bad copy of the game.

try downloading save games from net and load the game from there and check if the prob persistes
I opened up the console and it says some error in map file (pk001.004)..
The cheat code for jumping to a level does not work..
According to cheat websites, the cheat code for jumping to second level is aerodefense2.. but it doesnt recognize the command.
i am using command line argument also.. (+set com_allowConsole 1)
