CPU/Mobo Suggest AM3+ Mobo Between 4-5k

I own a AMD 960BE whose Asus M4A-88-T (USB3) died few days ago. I am planning to RMA the motherboard but unable to find the bill.

In the meanwhile I need a new motherboard for the system. I am out of sync with the knowledge of latest motherboards so please suggest based on the below requirements:
- AM3+ socket (AMD 960BE processor)
- Need onboard graphics (mostly for HD movies and casual gaming) and HDMI port
- Need USB 3 ports
- Preferably with Sata 6gb/s
- suggest company that supports RMA by courier (pc used at Thane, Mumbai.)
- Budget within 5k.

Also, I want to know what difference does ATX or mini ATX or micro ATX size makes practically.
I own a Cooler Master 310 cabinet. Wont they all fit? So the smaller the better right? Or is there something else to consider?

Please help me. I need to buy one (online) asap.

Gigabyte GA-78LMT-USB3 or Asus M5A78L-M/USB3. Both don't have SATA 6.0Gbps though.

ATX boards have more space for more PCI/PCI-e expansion ports - usually 5 or more, while an mATX board will be shorter and have 3 or so PCI/PCI-e ports. The CM 310 supports all sizes upto ATX.
