Actually faced a similar problem with my Mi Max last year. My phone was ever so slightly bent, which resulted in the SIM card getting slight scratches on it. I got a new SIM and the problems vanished. It may be the same with you. Check the SIM for unusual marks and scratches.

Yes, once per phone. Don't generate the barcode beforehand from your new device. They are rejecting many barcodes as their system says it has already been claimed. It is better to generate right in front of them.

I am using Mi Max for almost a month now, if you are not bothered with the size, then go for it. Also, the hybrid sim slot is a negative for it.

It probably is the cashback option. I haven't seen it used here in India much (to be frank, I don't even know if that option works) But usually how cash back works is - you swipe for more than your bill amount and receive the rest as cash from the retailer.
