Well, I do not think this is just limited to Mumbai.

Coming to this incident, FOB (Foot Over Bridge) in this station is very much bottleneck-ed since quite long. Additionally, heights of steps constructed is uneven on Parel side. Similar tragedy can not be ruled out at other central railway suburbs like Dadar, Kurla, Ghatkopar, Thane and might be Kanjurmarg.
 
Its the same everywhere in India. Most of the accidents on roads and otherwise do not deserve to be called accidents. Its pure neglect on the part of people and could easily be avoided.
 
Don't blame the people alone. Everyone seems to love criticizing the brits and their colonisation these days, but the fact remains that mumbai is chugging along on the ancient infrastructure they set up. neither the bjp nor the congress or the shiv sena gave a rat's ass about improving or enhancing it while the population grew by leaps and bounds. this is why talk of the bullet train and statues has me especially irritated.
 
^^ That is true of many places. If not for the British, we would probably not have telecom or rail connectivity in a vast number of places. We are still dependent on infrastructure that the British setup in India.

But as far as mishaps like this and most accidents are concerned, I totally blame the carelessness and indifference of people.

Just today, I had a really close shave. All because an idiot driver of a car was trying to cross the road illegally (just a small opening in the divider) driving very fast while talking on the phone and without even honking the horn. Infrastructure may be part of the problem, but the biggest problem is the people themselves. Even in a city like Chandigarh with well maintained roads, accidents due to recklessness and carelessness are pretty common.
 
People lack discipline here. Children should be taught discipline in childhood even if it means using a stick to beat them. We should go Singapore style and beat people with a cane for spitting or littering or jumping out of line. If there was a queue system for mumbai locals, getting in and out of trains would be much easier for everyone. Right now, its like a group of hooligans running and jumping inside trains even when the train hasn't come to a stop on the platform. Add to that the constant shoving of people by train groups to make space for their every day buddy is even more of a headache.

Mumbai needs atleast 6 monorail tracks from Churchgate to Dahanu or Palghar (or where ever the last stop is) over the present 6-8 tracks on which the locals run to reduce over crowding. The main area where the govt lost is all the stations have private commercial buildings right next to the railway stations such that railways cannot expand stations wider.
