Do whatever you can from the above. Plus start writing a private journal(Diary) and obviously regularly. Mention each and everything that happened the following day. Gradually this will improve your writing skills and your English. Do not write in Hinglish. Like,"You know what diary? Aaj main aaloo kharidne gaya". Proper English. You will see the improvement.

The best way to make a language "native" is to start thinking in that language. You will have difficulities otherwise because you will be thinking in your mothertongue's idioms, and then translating that to English.

Many idioms in my/your mothertongue do not have equivalents in English, and similarly many idioms in English will have no equivalents in your mothertongue.

You need to both speak in English and hear others speak English to become fluent. For the hearing part, i'd suggest going with Audiobooks.

Librivox is a good place for free audiobooks.

Get Rosetta Stone.. the best way to learn a language apart from other methods already mentioned (books, thinking in English etc)
 
You need to understand the language first and build up your vocabulary, which you already do, so perfecting it must be easy. Now all that's left is practice, especially hearing and then speaking. Don't be too bothered by mistakes, learn from them as you go. I think the advise given above suggesting to start thinking in English instead of your native language is a great one.
