as you all know I suffer from Duchenne Muscular Dystrophy (Currently not Curable) because of which I can't walk & my hands have very little movement

my right hands palm works fully but my full left hand excluding 3 fingers has stopped working

I can play RTS & RPG games with just a mouse & FPS with voice commands via game commander 2

now the problem arises in sports & racing games

so either I need a small thing which has only the arrow keys of a keyboard

or

a software via which I can assign the A/D keys or Left/Right Arrow keys of the keyboard on the X Axis of the mouse & the W/S keys or Up/Down Arrow keys of the keyboard on the Left/Right Mouse Buttons

I cannot use the full keyboard, gamepad, joystick or wheel sadly :(

i think you can configure the mouse as a game controller and use the x and y axis as forward and sidewards movements
 
Go to Options ---> Controllers --> see if they have an option for setting as mouse for input.

GTR 2 has this I think...

only GTR2, Live for Speed, Midtown Madness, Midtown Madness 2 & the Moto GP Series has that

The prob is I hate those games

NFS Series & Test Drive Series only
 
Ummm....don't know about axes, but I thinnnnk you could use Autohotkey software to remap mouse middle button for W, right-left buttons for A-D and one of the mouse scrolls (scroll up/down) for S.
