    No overclocking? What's the ambient?? Unless the ambient is 50C (which is very unlikely), there's a problem. Reapply TIM and check whether H60 is properly over your processor. If everything is ok, then submit you H60 for RMA.

    Check the front USB2 header. Reconnect it at the motherboard. IF the problem continues, then check the header at the chassis. If the problem continues after that, then it's the problem with chassis USB header. Time to replace it or the whole chassis - depending on the chassis & structure.

    For a 4 port USB3 card a PCI-e 2/0 X5 or high card is recommended. A PCI-E 2/0 X1 card can't provide enough bandwidth to 4 USB3 ports.
