Need to fill a PDF File. How to do it?

Hi guys,
I have to fill a document for my client. The problem is that it is in PDF format, and even after hours of searching I couldn't find a Word file for the same. The document which I want to fill has been attached. Please suggest a method to fill the same or somehow convert it to Word so that it becomes fill able. I tried the various Online PDF to Word converters but they couldn't manage the various boxes and stuff within the document. 'Able to Extract' came close but failed. I was wondering whether I should give Scansoft Omnipage a try. Or if you guys have any software, please try to convert the attached document and see if your getting a near-about copy which is fillable.

It's quite urgent as the document has to be submitted ASAP. Please help !

I haven't used any free tool but Foxit phantom or Foxit PDF editor works great.
You can directly edit the PDF.

If you can buy it , it would be great else you can get it free on torrents.
